Background Obesity and type 2 diabetes mellitus (T2DM) are associated with accelerated brain atrophy, white matter microstructural disruption, and resting-state functional network dysconnectivity in elderly adults, driven by converging vascular, neuroinflammatory, and insulin-resistance mechanisms.
